The Passport Program

The Passport Program is a reimbursement program funded by the Ministry of Children, Community and Social Services (MCCSS). It assists adults (18 years of age or older) with a developmental disability in accessing services and supports within their communities.

The Passport Program is a reimbursement program funded by the Ministry of Children, Community and Social Services (MCCSS). It assists adults (18 years of age or older) with a developmental disability in accessing services and supports within their communities. Individuals and families have the option of receiving funding to purchase services or they can choose to access services through Ministry-funded agencies. Participation supports can include programs, classes, camps, pre-employment, employment supports, and transportation to and from activities among others.

The program is also an aid to unpaid caregivers of those with a developmental disability, to obtain respite services and supports.

Support Provided Through The Program:

  • Provides program funding information to families and individuals within Simcoe County
  • Assists families with funding documents and planning
  • Provides program accountability to the Ministry of Children, Community and Social Services (MCCSS)

Passport Program support through Catulpa Community Support Services is available to Simcoe County adults (18 years of age and older) who have been successfully referred through Developmental Services Ontario (DSO), Central East Region.

Funding is provided by the Ministry of Children, Community, and Social Services. There is no fee for this service.

Passport recipients may choose an individual, agency, or organization to act as a broker to manage their Passport funding on their behalf. Some examples of the types of services that may be provided by a broker include assistance with budgeting, submitting invoices to a Power of Attorney (PA) on the recipient’s behalf, recruiting and scheduling workers, etc. Brokers may charge a recipient a brokerage fee of up to a maximum of 10% of the individual’s funded amount.

Referral to the Passport Program is only facilitated through Developmental Services Ontario (DSO), Central East Region. Access to funding under the program is based on available resources. DSO will confirm eligibility for adult developmental services and supports under the Services Inclusion of Persons with a Developmental Disabilities Act, 2008, and complete the DSO Application package for eligible applicants.

If you are already enrolled in the Passport Program and receiving support through Catulpa Community Support Services, please contact the appropriate Passport Worker based on your last name.

Simcoe County Community Action Program for Children (CAPC) and MotherCare (Canada Prenatal Nutrition Program – CPNP) are a part of a national program with then 25 years of experience supporting families in this region. We support pregnant participants, young parents, and Newcomers to Canada with children 0-6 years old who are currently living on a lower income.

Please confirm that you are currently living on a lower income (less than $40,000 before taxes per year) in Simcoe County and that you are currently pregnant and/or live with children 0-6 of age.
Data Collection Consent and Required Information
We do not maintain participant “notes” related to your participation with us but we are required by our funders to track attendance and the resources we distribute. We use an electronic data management system to record and store minimal personal information and program attendance. This information includes: name, date of birth, income, due dates and/or dates of birth of your children. Other information that we request is optional for you to provide. Detailed information about this consent can be found in our Welcome Letter.
