Crisis Response Network Coordination Program

Crisis Response Network Coordination provides individuals experiencing a situation or event that places them at risk with short-term support to resolve their immediate needs and mitigate more intensive interventions.

Access to support through this program is not limited to a waitlist. A Crisis Response Coordinator will work immediately with the individual to develop a plan to address real-time concerns for safety and well-being. This unique and supportive role also understands the Developmental Services system, which is helpful for families and individuals new to the area, new to the system, and unaware of what can be offered in their community. The Crisis Response Coordinator also works closely with several community service providers, allowing for increased knowledge of available resources to alleviate risks for an individual as quickly and safely as possible. 

Support Provided Through The Program:

  • Case Management
  • Respite or Interim residential support
  • Access to Funding
  • Referrals for psychiatric support and future planning   
  • Obtaining supportive housing and funding to participate in the community
  • Referral support to other community programs as requested
  • Participate in hospital discharge planning to safe environment for at-risk clients
  • Advocate and work collectively with community service providers, including police and Canadian Mental Health, Shelters, to reduce harm and risk for individuals who may be experiencing homelessness

Crisis Response Network Coordination provides support to Simcoe County youth (16 years of age and older) or adults (18 years of age and older) with a dual diagnosis of a developmental disability and mental health need, and/or challenging behavior who are experiencing a crisis.

Youth (16 years of age and older), with a dual diagnosis, who are involved with the justice system may be eligible to receive up to four months of case management support to assist them with legal services through Crisis Response Network Coordination

Dual Diagnosis: Is defined as a co-existing developmental disability and mental health needs and/or challenging behaviors.

Funding is provided by the Ministry of Children, Community, and Social Services. There is no fee for this service.

Simcoe County Community Action Program for Children (CAPC) and MotherCare (Canada Prenatal Nutrition Program – CPNP) are a part of a national program with then 25 years of experience supporting families in this region. We support pregnant participants, young parents, and Newcomers to Canada with children 0-6 years old who are currently living on a lower income.

Please confirm that you are currently living on a low household income (less than $40,000 before taxes per year) in Simcoe County and that you are currently pregnant and/or live with children 0-6 years of age.
Data Collection Consent and Required Information
We do not maintain participant “notes” related to your participation with us but we are required by our funders to track attendance and the resources we distribute. We use an electronic data management system to record and store minimal personal information and program attendance. This information includes: name, date of birth, income, due dates and/or dates of birth of your children. Detailed information about this consent can be found in our Welcome Letter.

Le Programme d’action communautaire pour les enfants (PACE) et MotherCare (Programme canadien de nutrition prénatale – PCNP) du comté de Simcoe font partie d'un programme national qui soutient depuis 25 ans les familles de cette région. Nous aidons les participantes enceintes, les jeunes parents et les nouveaux arrivants au Canada avec des enfants de 0 à 6 ans qui vivent actuellement avec un revenu annuel limité.

Veuillez confirmer que vous vivez actuellement avec un faible revenu du ménage (moins de 40 000 dollars avant impôts par an) dans le comté de Simcoe et que vous êtes actuellement enceinte et/ou que vous vivez avec des enfants âgés de 0 à 6 ans.
