Summer 2010 Camp Registration Process

 

* Important - Please read instructions carefully for your child to receive support. 

New Campers

If your child did not attend a summer camp included on our partners list in 2009, please refer to the link to the specific camp you wish your child to attend and complete the following:

• Form identifying need for inclusive services
• Consent form to allow for planning and open communication between your child's camp and Catulpa Community Support Services.
• Recreation Profile Worksheet

Please sumit the completed inclusive services request package to your recreation provider to allow for the preparation/accommodation of the participant either by mail, person or e-mail.  Please note spaces are limited.

Once forms are received by your recreation provider, the camp will be in contact with you to confirm availability and registration details.

Please await confirmation from the camp prior to registering your child.

 

Returning Campers
