Children's Case Coordination Services for Simcoe County
Children's Case Coordination Services provides information, consultation, and community case conferencing for children/youth, up to the age of 19, and their families who have complex needs requiring special attention and creative service planning. A special needs child is one who needs assistance in meeting complex behavioral, developmental, emotional and/or physical needs. Cross Ministerial involvement is often required. The child’s needs exceed the ability or mandate of one agency or sector to meet or have service needs requiring long-term care.
Children’s Case Coordination offers three levels of service:

Information Sharing: an individual can contact the coordinator requesting assistance in identifying available services. Helps families gain access to services that are most appropriate to respond to their needs.

Consultation Services: If more depth involvement is needed to explore alternative means of service provision, the Coordinator is available for consultation services. The Coordinator can meet with the child, family, professional or agency (legal consents have to be in place) requesting assistance in obtaining, identifying and/or coordinating specific services.

Case Conferencing Process: Case Conferencing is available if the child's/family's needs cannot be addresses through the level of information sharing and/or consultation services.

Referrals can be made by the legal guardian, youth, or referring agency; consent is required. Direct contact is then made with the family, whether through a home visit or office appointment.

The Coordinator gathers all relevant information to complete a service plan assessment based on the child's strengths and needs as well as service involvement history.

Parents, agencies and professionals are then requested to attend a case conference to address the child's/families needs. A service plan for each child is developed, incorporating the inter-agency service provision and role each individual involved will play.

A primary worker or the parent is identified as the community case manager.

The Coordinator chairs the Case Conferences and distributes minutes to all participants.

The Coordinator provides follow-up case conferences at specified intervals to monitor the child's progress.
Children’s Case Coordination assists the Service System by:
- Assists in planning, research and system development.
- Helps identify gaps in the service system
- Heightens awareness of the availability, nature and value of children’s services
- Link families who require services that may not be available in Simcoe County but may be available and appropriate within the Central East Region.
